The HOME Team provides street-based outreach across Portland, engaging people who are unsheltered and often disconnected from care.
We focus on relationships first. Not paperwork. Not pressure.
Sometimes the first step is small. A conversation. A ride. A meal. Over time, those moments become trust — and trust creates the opportunity for change.
We don’t wait for people to come to services. We go to them.
What HOME Team Does
Engagement
Building trust and connection through consistent, respectful outreach.
Basic Needs
Providing food, clothing, and other immediate support and connection to supports.
Connection to Care
Linking people to Milestone services and community resources when they are ready.
Emergency Transportation
Offering emergent rides to detox, shelter, medical care, and other appointments.
This work isn't simple. But it works.
People don’t say yes the first time. Or the second.
Many have been failed by systems, turned away, or lost trust.
HOME Team shows up anyway.
Again and again.
Because change doesn’t happen on our timeline.
It happens when someone is ready — and knows someone will be there when they are.
What HOME Team Doesn't Do
